Park City Clinic - Out of State Policy

We ask that you pay in full at the time of service.

Park City Healthcare Billing Office will be sending out a red and white billing form to the mailing address we have on file for you. You will not be able to receive this billing form today. This is the form that you must submit to your insurance company for reimbursement. Please do not submit the itemized statement you are given at our check out desk. Your insurance company will not pay from that statement.

Insurance companies may send the reimbursement check to you or to our Billing Office. If they chose to send the reimbursement check to Park City Healthcare billing, our billing office will reimburse you.

If you have Medicare of Tricare, none of the above applies. We will bill Medicare and Tricare for you.
